I have this as well, when I first started playing Sims 4 only about 2 years ago properly with an infant it'd happen occasionally as you'd expect but in recent months it's every 2 minutes, even after fully filling the toddler needs, I'll log fill the adult Sims needs, only for them all to cancel off and check toddler will replace everything.
Console mainly player, no mods/cc of course :)
