Sims will suddenly go 'Out of the World' to infinity

I have the same problem. Sometimes my sim goes to the bathroom and then they start walking straight out of the world infinitely. To fix it I reset them, but sometimes they appear with the censoring mosaic, so I have to direct them to use the bathroom again to take the mosaic off.  This happened to me at least three times, and the first one was around the Grim Missions before Life and Death came out. 

I play with mods, but I see that this also happened to  people who don't use them, so I hope we got a fix someday. 

Happened to me 3 times in Newcrest. If i focus the camera to them, it seems that they are in the canal/river? But I can’t actually see them. They then can’t get out and are just stuck there no matter what. This has also happened to my sims cat, and ‘call over’ didn’t do anything.

Only loading to the main menu and loading back in to the save seems to fix it, and since i can’t see the sim when it happens, I can’t reset the sim.

Obviously there is no way they could have walked there, they have just glitched down there. I haven’t actually seen them go there, they just sometimes end up there when i’m controlling another sim. This has also happened to my sims cat, who seemed to be in an inaccessible bush, and ‘call over’ didn’t do anything. I don’t know if this happens anywhere else, I have only experienced it in Newcrest.

Only loading to the main menu and loading back in to the save seems to fix it, and since i can’t see the sim when it happens, I can’t try resetting the sim.

My sim is an elder- and besides grabbing dirty dishes and pouring it into a bake tray to make a new meal- they're pretty okay bug wise.

(strange behavior - for sure a bug, but i feel it adds a funny element to this elder sim so idc.)

However:

 I've caught them walking through walls/walking through fences/walking across debug objects and over entire water features that are not interactable normally to sims --> to the edge of the map where they would have disappeared. (walking in a straight line)

I was able to pause the game and quickly type in [cas.fulleditmode] to Shift -->debug(reset object). before they disappeared into a mountain range and I'd be unable to find them. It did teleport my sim back into their house however I was worried this would be a reoccurring problem for this sim.

I do have images of this saved via [C] to capture this happening - this was in Newcrest, near the twin points(?) smt property.

(by the kids pirate ship playstructure/gyser that's fenced off - there are no fishing locations on this side of the map just sidewalks). 

This happened to me on two separate occasions in two different worlds. I don't know what the correlation was because I had the sims doing different things. The first time the sim just walked right off the map. The second time I was able to stop the sim by resetting him. The teleport cheat works also. I have no mods, but first noticed it after I bought the Horse Ranch and For Rent expansion packs. 

I have no mods so this is clearly not a mod issue. 🤷

I have encountered the same issue/a similar issue.  Sims walk off the map and continue infinitely. Initially, I experienced this while using mods but, as I had a save one minute before, after reloading the game without mods, the issue occurred again immediately.

Steps to Reproduce:

1. Directed a Sim to use the bathroom at a lot in Granite Falls.
2. The Sim walks towards the bathroom, then suddenly turns around and faces a cardinal direction.
3. The Sim begins walking through walls, objects, and terrain directly, continuing infinitely without following the usual path.
4. The Sim eventually disappears from the map.
5. Right-clicking the Sim’s portrait brings the camera to the furthest point in that direction, showing no objects or Sims.

Expected Behavior:
The Sim should complete the action of using the bathroom, without walking through terrain or disappearing off the map.

Actual Behavior:
The Sim walks through objects and terrain, continuing indefinitely. Cancelling the action of using the toilet doesn't resolve the issue. Instead, the Sim queues up actions with objects on the lot (e.g., sleeping on a bed). While the Sim appears in a sleeping state, right-clicking on the portrait shows the Sim past the boundaries of the map, and the bed remains empty.

Additional Information:

• The issue occurred both with and without mods, so it seems to be unrelated to the mods used.
• The problem happens immediately after reloading the game without mods.

Happened to me today. He kept walking until he stepped off the map and into outer space.  I finally had to "resetsim" and then use "teleport here". The funny thing was that his feet kept moving in Outerspace, but he went nowhere. 😆

This should have an active AHQ Bug report but I can't find it atm.
