Although, I think they should implement some/many mod ideas, for example I love the lock skill mod (to prevent selected skills from being learned per Sim, forget skills or lock in place at current skill level) but it has faults since it isn't an official feature (constant notices of learning a skill when you haven't as you have locked it)—but I don't think they should break the teen rating nor make it have too much realism.
However, I do think if they would implement features that are pretty much of existing mods they will be careful in that sense. And many of the mods are long time wants/wishes/request.

I mean its u can die from sadness in the game but abortion or miscariage change the rating makes no sense

I don't think EA decides what is "Teen" rating they just abide by it, and I don't think they'd change the rating of the Sims franchise just for these select topics to be added. Ratings are determined by a third party anyways, and EA follows it. So opinions and debates on that matter would just be moot, and trying to make sense of it would just be self defeating.
