Disappearing kids

Steps: How can we find the bug ourselves? Have a Sim have a baby and attempt to age them up to toddler
What happens when the bug occurs? I've had two kids disappear! No social worker came either time to take them. The first time, my child was just in their crib and I looked over for them and they were gone! No evidence of them ever existing (no relationships with other Sims). No warning about being taken either. The second time I tried to age up my new baby to a toddler, tried to change their hair, then he was gone! Just into thin air! No evidence of them existing either, both kids disappeared from icon bar. I have mods installed and expanded the household to 12 people. This is the only reason I can think of why this may be happening.
What do you expect to see? Baby remains in icon bar, ages up properly.
