IvyYuuki The type of Sims 4 error in your dxdiag is pretty generic, but the main cause aside from ones you've already addressed (mods, a broken save) is an issue with the graphics driver. However, your dxdiag crashed while running as well. This could be a random fluke, but it could also point to a more general system issue. So please run a couple of basic checks of your Windows system files:
- Click Windows key-X and select "Powershell (Administrator)" or "Windows Terminal (Administrator)" , whichever you see
- In the window that appears, copy and paste "DISM.exe /Online /Cleanup-image /Restorehealth" without quotes, and enter
- The system will take a little while to validate; if it throws an error, please list it
- Otherwise, copy and paste "sfc /scannow" without quotes into the window, and enter
- Post the message you receive
Restart your computer, open Windows Settings > Update & Security, click the box to check for updates, and if any install, restart again afterwards.
You can try again to play, still with a clean user folder, but if that doesn't help, I'd suggest doing a clean uninstall and reinstall of the graphics driver. Here's how:

Rather than using the very newest driver from Nvidia, try a slightly older one. You can always update later, but going backwards would require another clean uninstall.
