[COLLECTION] Issues with Thumbnails/Icons/Screenshots

Please report everything concerning missing/wrong thumbnails/icons in here.  There's a few exception that go in other threads. See below for links. Don't worry if you post in the wrong thread. I'll move it accordingly.

Create one post per missing thumbnail.
