I have the same issue. Have tried all suggested solutions and have also raised a case (126278946). This has been incorrectly marked as "Fixed" (so I reopened the case) - I've heard NOTHING from the support team at all - it's really shockingly poor. How can I contact the support team/someone who can help directly? I've tried tweeting, emailing, responding on the case directly. Case now says "In Progress" but the LEAST you could do is give me an update - after all, you have my money and I have no product.
TBH this is not the first issue I've had - previously my EA account was accidentally linked to a different PSN account, and when I switched back this resulted in my losing hundreds of pounds worth of content. You were really no help at all in resolving that either. Hopefully someone will respond on this as otherwise I'm out of ideas as to how to actually get you to do anything at all about this. Please respond.
