Hey, how does issue 1 manifest? Sometimes you'll be kicked to the title screen, but you should be able to join straight back in, then review your post-match summary.

As for issue 2, can you try playing after restarting the system with a Clean Boot, and see if that helps stability?

Updating and clean reinstalling drivers can help if you're having freezing/stability issues, but such issues can be tricky to lock down. 

For issue 3, Do you happen to have a clip of it. Did your teammates fully die, and did it force you to the summary page? Did you get the early leaving warning?
