@TS_ZeroCool I see you have set adaptive Resolution FPS Target to 0 however yo ucouldtry to limit the fps to 140fps @ 144Hz and see if this helps.
Also have you gone to display settings>Advanced display settings>display adapter properties>Monitor on your pc and changed "Screen refesh rate" to 144 Hertz?

@EzyHawk Thanks for the reply! I do have my monitor set to 144hz both in NVIDIA display settings and the monitor itself.

Setting to 144 fps doesn't help at all, and in fact makes it worse.
Can anyone else advise? I am still constantly getting bad FPS spikes and yet to experience any smooth gameplay with an RTX 2080. This is rather pathetic.
